A quiet hospital corridor with daylight falling across the floor

For professionals

You have a bed to clear and a family who has not started looking

We are a local placement service in Anaheim. Call us with a level of care and a discharge date and we will come back the same day with which licensed homes have a bed and which can legally take that level of need. No cost to the patient, and none to your facility.

(714) 975-9652

Open every day, 8am to 6pm, including weekends.

What you get

Four things that matter at four o'clock on a Friday

A same-day shortlist, not a database

Usually four or five homes out of the 97 the state licenses in Anaheim, with the reasons the rest were ruled out. We have been inside them and we know who has a bed this week.

Weekends

Discharges do not wait for Monday and neither do we. We answer seven days until 6pm, which is more than most of the services your families will have already called.

A straight answer on level of care

If the need is beyond what a residential care facility may legally accept or retain, we will say so and decline the referral rather than move the problem to somebody else's building.

Spanish, and language matching

We work in Spanish, and in parts of the county we can find homes where overnight caregivers speak a resident's first language — which matters more in dementia than most discharge summaries capture.

Use this whether or not you ever refer to us

The licensed register is public, and it is free

We publish every care home the state licenses in Anaheim, with licence numbers and bed counts. No login, no form, no catch.

97 licensed homes, 2,241 licensed beds, 83 of them six-bed board and care. It is searchable and filterable by area and by size, and every entry carries the state licence number so you can pull the facility record yourself.

We also publish it as structured data if it is useful to anyone building a resource list.

Several discharge planners use it simply as a reference. That is genuinely fine, and it is the point: we would rather be the people whose list you trust than the people who call you every Tuesday.

So there is no ambiguity

We offer nothing of value for a referral

No gifts, no lunches, no sponsorships, no payments, no marketing support, nothing. Not as a policy we might flex, and not because we are frugal. Inducements tied to patient referrals create exposure for the referring professional as well as for us, and no amount of volume is worth putting a case manager's licence anywhere near that.

If somebody else in this industry is offering your department something, that is worth noticing.

What we will do is answer the phone at the weekend, give you a straight answer about level of care, and tell you when the right answer is a facility we earn nothing from.

To move fastest

What to have in front of you when you call

None of it is essential. All of it saves a day.

  1. 1Current level of care: transfers, continence, mobility, one person or two.
  2. 2Cognitive status, and specifically whether the patient exit-seeks or wanders.
  3. 3The discharge date, and whether it is firm.
  4. 4Payer: private pay, long-term care insurance, Medi-Cal, VA, or unknown.
  5. 5Any condition that limits an RCFE placement — a feeding tube, a trach, stage 3 or 4 pressure injury.
  6. 6Which family member is actually making the decision, and their phone number.

Common questions

From case managers and social workers

Is there any cost to the patient or to our facility?
None to either. Families are never invoiced, and we never ask a hospital, skilled nursing facility or hospice for payment. The receiving care home pays a referral fee when a resident moves in, which is the standard arrangement across the industry.
Do you offer anything in return for referrals?
No, deliberately. We do not provide gifts, meals, sponsorships, payments or anything else of value in exchange for a referral, and we will decline if offered the reverse. Anything else would risk both of us under federal and California law, and it is not how we want to compete.
How quickly can you turn a referral around?
Same day in most cases. If you call before 6pm with a level of care and a discharge date, we can usually come back the same afternoon with which homes have a bed and which can legally take that level of need.
What if the patient needs skilled nursing rather than residential care?
We will tell you, and we will not take the referral. Placing somebody into a facility that cannot care for them within the limits of its licence is a misdemeanor in California under Health and Safety Code section 1569.47, so this is a line we hold regardless of what it costs us.
